I was going to wait until I was done my shredded tshirt to give a tutorial but here it is anyhow (I saw "State of Play" tonight instead - worth it!):
Step 1: Get a shirt. Don't be getting some sort of wool or insane fabric like that. Stick with some regular cotton shirt and you'll do fine.
Step 2: Cut off the hem of such shirt. Try to be as straight as possible because I found it somewhat difficult if your hem is all over the place.
Step 3: Using either a seamripper device, a pencil, or your fingers, slowly begin pulling at the end of the fabric. You should see strands of the horizontal parts of fabric begin to come out/apart. They may be extremely far apart and very loose but the further you get into the shirt the more normal it becomes.
Step 4: Just continue pulling, basically. This isn't so much difficult as it is time consuming. What I did was I would put my left thumb and index finger underneath the section I was pulling at and pull with my right hand so it came apart easier as opposed to just ripping it.
Step 5: If a hole appears it's not a big deal. If you don't want holes, don't pull as hard. Go as high up as you want and do it wherever on the shirt you want - it's really your choice. I believe you can machine wash them (I think!) but if you want to be extra-careful just hand wash it and hang to dry.
